Intent to claim a Health Care Card
To get the card, first you will need to register an 'intent to claim'. This will let us know that you will be applying soon. An 'intent to claim' is automatically registered if you make an online claim. Online claims are currently available for the Foster Child Health Care Card and the Ex-Carer Allowance (child) Health Care Card on line.
If you wish to claim a Health Care Card using a paper claim form, you should still register an Intent to Claim. You can do this in a number of ways:
- online - go to the Register for a payment or service section and select either 'Register for yourself' or 'Register for someone you know'
- by telephone - call us on one of our services and payments telephone numbers
- in person - visit us at your nearest DHS Service Centre
You will then be sent a claim form in the mail. Once you have registered your 'intent to claim', the claim should be lodged within 14 days.
This will ensure you get your card on the earliest possible date, if you qualified at the time of registration.
