Application for Child support assessment form (CS1665)
Use the online form to apply for a Child Support assessment if you're a new or existing customer. The online form easily guides you through the application process and there's a help feature if you need more information. If you can't answer all the questions, you can save your application and finish it later. You can also fill out the Parent Referral Guide to have tailored products or services posted, or a list of them emailed to you. If you're using our online service you can fill in and submit the form using Child Support Online Services.
