This form is used when a customer needs to provide confirmation that they have actively participated in a program/s of support as part of their claim for Disability Support Pension. The form is designed to be completed by the provider/s who delivered this program/s for the customer. Information provided in this form will assist in assessing the customer's eligibility for an income support payment.
