The Centrelink Medical Certificate along with other medical information is used to help Centrelink make decisions about customers with participation requirements.

Supplies of Centrelink medical certificates are only provided to registered medical practitioners.

To re-order Medical Certificate forms e-mail the information requested in the re-order form supplied in the back of Medical Certificate form pads or fill in and post the re-order form supplied in the pad.
