IVF/GIFT program

The Pharmaceutical Benefits Scheme subsidises a range of in-vitro fertilisation (IVF) or gamete intrafollopian transfer (GIFT) treatments.

You can now order in-vitro fertilisation (IVF) and gamete intrafallopian transfer (GIFT) medicines online through Health Professional Online Services (HPOS).

The Medicare Benefits Schedule item codes and descriptions for IVF/GIFT treatments covered by the Pharmaceutical Benefits Scheme (PBS) are explained in the following table.

Lodging an application

The IVF/GIFT summary of order can be lodged either online or by post.

Online

You do not need to send the patient forms to us if you have submitted the IVF/GIFT summary of order form online. For audit purposes, you need to complete the patient form and keep it for at least 2 years from the date of supply.

Post

The corrifollitropin alfa application form must be completed for patients being treated with corrifollitropin alfa. Details of up to 3 patients can be provided on 1 form.

  • the IVF/GIFT Elonva® applications form (PB004) must be completed for patients receiving treatment with Elonva® as described in item 13200, 13201, or 13202 of the Health Insurance (General Medical Services Table) Regulations. Send the form to the IVF/GIFT address on the contact us page
