Completing Centrelink forms using HPOS

A guide for health professionals to help complete and submit medical forms in HPOS which will update your patient's Centrelink record.

You must have an individual PKI certificate to submit Centrelink forms through HPOS. PKI certificates ensure that information you submit through our online services is secure. Read more about PKI certificates, including how to get one.

The 2 forms you can complete and submit to Centrelink using HPOS include the:

  • Medical Report - Disability Support Pension form, which can be used to support your patient's application for Disability Support Pension, and
  • Medical Certificate form

Once you submit these forms through HPOS your patient's Centrelink record will be updated.

Any changes you make to your provider details will not be saved through this form. To permanently update your provider details you will need to do this through the Provider personal details option within the HPOS main menu.

Create and submit Medical Report - Disability Support Pension form

  1. select Centrelink Forms from the main menu in HPOS
  2. select Medical Report - Disability Support Pension option from the page displayed
  3. instructions for the form and a declaration will display. You must accept the declaration before you can Continue
  4. enter your Patient Details, select Save and continue
  5. responses to the Significant Impact Medical Conditions question determines which subsequent questions you need to answer. Mandatory fields are required to be completed before moving to the next page. Once complete, select Save and continue
  6. responses to the Minimal or Limited Impact Medical Condition(s) question determines what information needs to be entered. If the patient has medical conditions that cause minimal or limited impact on their ability to function, the details of the diagnosis and treatment must be supplied. Once complete, select Save and Continue
  7. if you would like to provide a medical certificate with this report, select Yes and give the details of the medical certificate in the certification section. If you do not want to provide a medical certificate, select No. Once complete, select Save and continue
  8. select No or Yes to the Release of Medical Information question. Select Save and Continue
  9. the Provider Details page displays the Confidentiality of Information and allows you to check your details. You can select the Update option to amend your address or phone details if required. Select Save and continue
  10. the form is now complete. Check the information in the Review and Submit screen. If any details are incorrect, you can use the Update option for each heading of the form to go back and correct any details
  11. if all the details are correct, select Submit and the form is submitted. It will then update your patient's Centrelink record
  12. the form has now been submitted and the Success Screen is displayed. Select the View PDF option to print or save a copy for your records. Select Finish to close the form

Create and submit Medical Certificate form

  1. select Centrelink Forms from the main menu in HPOS
  2. select Medical Certificate option from the page displayed
  3. the Medical Certificate Instructions screen will be displayed
  4. read the instructions and select Continue
  5. enter the Patient Details. Select Save and continue
  6. enter the patient's primary Medical Condition(s). All fields are mandatory. Select Save and continue
  7. complete the Capacity to Work or Study details. All fields are mandatory and you will not be able to progress without this information. Select Save and continue
  8. next the Certification by Medical Practitioner screen displays the Confidentiality of Information and also displays the Authenticated user provider details. Select Save and continue
  9. the form is now complete. Check the information in the Review and Submit screen. If any details are incorrect, you can use the Update option for each heading of the form to go back and correct any details.
  10. if all the details are correct, select Submit and the form is submitted. It will then update your patient's Centrelink record
