Claiming the External Breast Prostheses Reimbursement

You can claim the reimbursement by completing and submitting the claim form.

You can claim the reimbursement by submitting a completed claim form.

If you currently receive financial assistance from the Department of Veterans’ Affairs, you should claim your entitlement through them.

If you don’t have access to a printer, you can call the Medicare general enquiries line and order a form or pick one up from a service centre.

Supporting documents

You’ll need to provide other documents to support your claim including:

  • a copy of your original purchase receipt - it must include a description of the purchase
  • any other documents we ask you for

If you submit your claim in person at a service centre, your receipt will be returned to you.

If you submit your claim by mail or in a service centre drop box, you can ask for your receipt to be returned to you by calling the Medicare general enquiries line.

Claim outcome

We’ll send you a payment statement if your claim is successful. If your claim is rejected, we’ll send you a letter explaining why.
