If you do not have access to a printer, you can pick up the form from your nearest service centre or call us on 132 717 and we will send one to you.

Supporting forms and documentation

You may need to complete some extra forms to help us to accurately assess your claim, based on your and your partner's individual circumstances.

When you claim online, the list of forms required will be presented to you once you submit your claim.

You will also need to provide identity documents and a medical certificate that your treating doctor can provide.

Submit your claim

Most people have their funds paid directly into their bank account. Make sure you register your bank details when you submit your claim so you can too.

If you have submitted your claim online, you can submit your documents online, which will attach your documents directly to your claim.

If you are unable to claim online, you need to return your completed forms, identity documents, medical certificate, and any other documents we have requested, to your nearest service centre or by post.

Claim outcome

We will inform you whether your claim is successful. If you are eligible for Sickness Allowance, we will tell you when your payment will start and how much you will get paid.
