You can identify as an Aboriginal or Torres Strait Islander Australian by completing the Medicare Voluntary Indigenous Identifier form below. You don’t have to do this. It’s your choice.

The information you provide helps shape future government plans and policies. It is used to work out whether these policies are making a difference.

We will keep your personal information secure. It is protected by privacy rules and laws. Private information is never used at the individual person’s level.

To identify as an Aboriginal or Torres Strait Islander Australian on your Medicare record:

  1. complete the details below and print the page, or you can print the page and fill it in by hand
  2. sign the form
  3. send the completed form to:
