Verify your identity for PRODA with your change of name certificate

How to verify your identity for PRODA with an original or a certified copy of your change of name certificate issued by an Australian state or territory.

To verify your identity through the Document Verification Service (DVS) using a change of name certificate, certain information from the certificate is required. This information can vary in location depending on the state or territory it was issued in.

You cannot electronically verify change of name certificates from:

  • New South Wales
  • Queensland
  • Victoria, and
  • Western Australia

Australian Capital Territory

You can electronically verify Australia Capital Territory (ACT) change of name certificates back to 1930.

Mandatory fields on all certificates are the registration year, registration number, date of issue, previous name, and new name.

Northern Territory

You can electronically verify Northern Territory (NT) change of name certificates back to 1987.

Mandatory fields on all certificates are the registration year, registration number, new name, previous name, date of issue and certificate number.

  • The registration year is in the fourth row in the second column to the right of the words Date and Place. Use the year noted in this date
  • The registration number is in the fourth row in the second column
  • The new name is in the first row in the second column to the right of the title name after change
  • The previous name is in the second row of the certificate to the right of the title name before change
  • The date of issue is on the left hand side
  • The certificate number is at the bottom of the certificate on the left hand side

South Australia

You can electronically verify South Australian (SA) change of name certificates back to 1990.

Mandatory fields on all certificates are the registration year, registration number, new name and previous name. Date of registration and certificate number are also required if the certificate was issued on or after 1 November 1999.

  • The registration year is in the fifth row in the second column. Use the year noted in this date
  • The registration number is in the fifth row in the second column
  • The new name is in the first row of the certificate in the second column
  • The previous name is in the first row of the certificate in the second column to the right of the words surname and given names
  • The registration date is at the bottom after the words ‘Given under my hand and seal this’
  • The certificate number is at the bottom beneath the seal

Tasmania

You can electronically verify Tasmanian (Tas.) change of name certificates back to November 2001.

Mandatory fields on all certificates are the registration year, registration number, new name, previous name and date of registration.

  • The registration year is on the top right hand side of the certificate and is the number noted after the forward slash
  • The registration number is on the top right hand side of the certificate and is the first number noted
  • The new name is in the first row in the second column to the right of the title name registered
  • The previous name is in the second row of the certificate in the second column to the right of the title birth name
  • The date of registration is in the third row of the certificate in the second column to the right of the Registration Officer. Use the entire date. Where the date is 12 July 2010, enter 12/07/2010
