Aged Care Entry Record form (AC021)

Use this form to advise the us that a care recipient has entered either permanent residential care or commenced receiving a Home Care Package on or after1 July 2014. Section 63-1 of the Aged Care Act 1997 requires that this form be completed.

You must complete the form within 28 days of the care recipient’s entry or commencement date. The information assists us to advise a care recipient about their fees and to calculate the correct subsidy and supplements for approved providers. This form must be completed and signed by an authorised person of the approved provider.

The information will not need to be provided again in the monthly claim form, unless the care recipient’s circumstances change. The Aged Care Funding Instrument (ACFI) should also be completed for residential care recipients so that the correct subsidy can be paid.
