National Authentication Service for Health Public Key Infrastructure Certificate for Healthcare Provider Organisations - Terms and Conditions of Use form (HW050)

In consideration of the accepting of receipt of the National Authentication Service for Health (NASH) Public Key Infrastructure (PKI) Certificate of Healthcare Provider Organisations (Application) the Organisation’s legal entity (in these Terms and Conditions called the ‘Organisation’) agrees in relation to any NASH PKI Certificate for Healthcare Provider Organisations (Certificate) provided to it on a compact disc (CD) in response to the Application.
