Avoid delays - use the latest HI Service and PKI registration forms

There are changes to evidence of identity for Healthcare Identifier (HI) Service and Public Key Infrastructure (PKI) forms.

You’ll need to use the latest HI Service and PKI registration forms. These forms came into effect on 1 March 2017.

The forms now include providing evidence of identity from a primary and secondary list of documents if you’re a new customer.

Use the new Help me find a form tool to find the latest version of any form. You can search by code or title.

If you’re using Health Professional Online Services (HPOS), you now need to use your PRODA account to access it. If you don’t have one, register for a PRODA account online now.
