It’s now even easier to get an Immunisation History Statement

Are you having trouble getting a statement? Your vaccination provider can now print one for you.

You might need an Immunisation History Statement for yourself, or your child aged under 14, for school, child care or work.

It’s easy to get a statement yourself. You can use your:

A young person aged 14 or older can get their own Immunisation History Statement. To do this they need to create a myGov account and link it to Medicare.

You can download, print or email your statement.

But what if you can’t access these options? You can now ask your vaccination provider to print an Immunisation History Statement for you.

We can also send you a copy but this can take up to 14 days. Just call our Australian Immunisation Register enquiries line.

If vaccines are missing from your or your child’s statement, talk to your vaccination provider.
