There’s a new Application for a Medicare entitlement statement form you need to email or post to us, using the address on the form. Your tax agent can help you do this.
You may be exempt from paying the Medicare levy if you meet certain medical requirements or aren’t entitled to Medicare benefits.

To help you get it right and avoid delays:
- use the new Application for a Medicare entitlement statement form
- attach certified copies of supporting documents with your form
- check documents are in PDF format and not password protected if you submit your application via email
If your tax agent is lodging your form, they can email us multiple applications for different claiming periods.
