Your consent

We may need to talk to your treating health professional about your medical evidence. They may ask us to confirm that we have your consent so they can talk to us. To do this complete a Consent to disclose medical information form.

If you don’t provide your consent, we may not have enough information to assess your eligibility for Disability Support Pension.

Other forms and documents

You may need to complete other forms to support your claim. Submit them with your claim or contact us if you need more time.

When you submit your claim, it’s important to give us all required medical evidence and supporting documents so we can assess your claim faster.

If you don’t give us all evidence and documents, we may not be able to assess your claim and may reject it. You’ll need to complete the following forms under the described circumstances. Page 25 of your claim form will help you work out which forms you need to complete.

If you're in psychiatric confinement, the institution should complete a customer in psychiatric confinement form.

Submit a claim

Return your completed claim forms, identity documents, medical evidence, and any other documents we’ve asked for:

  • by post to:

  • in person at a service centre

Don’t forget, if you’d like to get another payment while you wait for the outcome of your claim, let us know when you lodge your Disability Support Pension claim.

After you submit your claim, we’ll tell you if we need more information or if you need to attend an assessment.

Wait for the result

If you’re eligible, we’ll let you know when the payment will start and how much you’ll get.

If you don’t agree with our decision:

  • call us so we can check the details and explain the decision
  • if you still don’t agree you can request a review of the decision
