When your baby arrives

Use your Parent Pack to finalise your pre-birth claim or submit your a new claim. You can also enrol your baby in Medicare and register their birth.

 

Get your Parent Pack

You’ll get a Parent Pack from your hospital or midwife. It contains a Newborn Child Declaration. Make sure your doctor or midwife completes the Proof of Birth declaration on the back page.

If you have a Centrelink Customer Reference Number and Medicare card, include these numbers on the back page.

Keep in mind, this is not a claim form to get payments. We need the back page as proof of birth to finalise your pre-birth claim for:

If you misplace your Newborn Child Declaration, it can’t be reissued. However, you can still provide proof of birth with either:

  • a birth certificate or certified extract
  • other Australian Government documents that verify the child’s name and date of birth, such as a passport or naturalisation certificate

You can also provide proof of birth with a letter from the doctor or midwife who attended the birth, including home births. The letter must include all of the following:

  • the details of the child’s parents
  • your child’s name, date and place of birth
  • the doctor or midwife’s signature, qualifications and registration number

Register the birth of your baby

You need to have registered, or applied to register, the birth of your baby to get some family assistance payments.

You must register the birth of your baby with your state or territory government. The hospital will give you a birth registration form. Registering the birth is free. When you do this, you may wish to purchase a birth certificate for your baby.

Finalise your pre-birth claim

Give us your child’s proof of birth to finalise any claims you started before your baby arrived.

To submit your proof of birth online, follow these steps:

  1. Sign in to your Centrelink online account through myGov or the Express Plus Centrelink mobile app.
  2. Select Add newborn child in your online account or Add Child in the Express Plus Centrelink mobile app.
  3. Answer all the questions and attach a scanned copy or photo of your child’s proof of birth.

These services will only appear if we’ve assessed your pre-birth claim.

It won’t appear if we haven’t assessed it yet. You should complete your Newborn Child Declaration form in full. Then upload this with your child’s proof of birth using the Upload documents function in your online account or app. Read more about how to submit your documents online.

To get your child a Customer Reference Number

When we process a claim for your child, we’ll create a Customer Reference Number (CRN) for them. You can find your child’s CRN in your:

To enrol for Medicare services

When you finalise your pre-birth claim for family assistance payments, we’ll enrol your child for Medicare services. You don’t need to give us the proof of birth again.

We’ll add your child to your Medicare card, and send an updated card once enrolment is complete.

Enrol your baby in Medicare

If you didn’t complete a pre-birth claim, you’ll need to submit proof of birth to enrol your baby in Medicare. To do this, you must complete the full Newborn Child Declaration form and return it to us. This will:

Only the birth mother can register a newborn child for My Health Record using the Newborn Child Declaration. You can read more about registering a child for a My Health Record on the Australian Digital Health Agency website.

You can submit your completed form either:

  • at a service centre
  • by post - the postal address is on the Newborn Child Declaration

If you misplace your Newborn Child Declaration, you need to submit a Medicare enrolment form. You’ll need to give supporting documentation requested in the form, including proof of your child’s birth.

We’ll add your child to your Medicare card, and send an updated card once enrolment is complete.

Submit your claim if you didn’t do a pre-birth claim

You can start a claim after the birth of your baby. If you do this, you can submit your proof of birth online when you claim:

To claim and submit your proof of birth online, follow these steps:

  1. Sign in to your Centrelink online account through myGov.
  2. Select Make a Claim on your homepage, then Get started from the Families category.
  3. Select Apply for Family Assistance (including Paid Parental Leave).
  4. Select the payments you want to submit a claim for.
  5. Answer all the questions and attach a scanned copy or photo of your child’s proof of birth.

Check if you can get Parenting Payment

You can submit your claim for Parenting Payment after the birth of your baby.

This is an income support payment while you’re a young child’s principal carer.

Check if you need to contact us about child support

If you or your partner have a current child support assessment, you can add your new child to it. Contact our child support line to do this as soon as possible. We can only add the child from the day you tell us.
