Register to transmit online or change nominated user's details form (AC005)
Use this form to register, update access levels or deactivate users if you are an aged care provider or service who is already registered for ACOC.
This form must be completed and signed by an authorised person of the approved aged care provider or service.
ACOC is Aged Care Online Claiming.
If you are only deactivating users from the ACOC website, key personnel can send an email to the aged care liaison team with the user’s name, A or RA number, and deactivation date.
