Statutory declaration ROCS - overseas medical practitioner form (MO035)

Use this form if you need to declare for the purpose of ROCS where documentation isn't available to support eligibility for an overseas medical practitioner who was engaged in medical practice in Australia at the time the incident occurred.

The ROCS only applies to claims or incidents first notified on or after 1 July 2004.

ROCS is the Run off Cover Indemnity Scheme.
