Juvenile idiopathic arthritis - initial PBS authority application form (PB060)

Use this form for a patient younger than 18 starting initial PBS subsidised treatment with a bDMARD for the treatment of severe active juvenile idiopathic arthritis.

A paediatric rheumatologist or a prescriber under the supervision of a paediatric rheumatology treatment centre must complete this form.

bDMARD is a biological Disease Modifying Anti Rheumatic Drug.
