Changes to the Centrelink Medical Certificate form

The Centrelink Medical Certificate is now a PDF fillable form.
Being a fillable form means you can download it from the Health Professionals forms list and either:
- complete it on your device
- print and complete it by hand.
You can use this form if your patient has a medical condition that may stop them from meeting their requirements.
You can also use it if your patient has a job but can’t work due to an illness or injury.
You can still order the Centrelink Medical Certificate forms.
Order Centrelink Medical Certificate forms either by:
- emailing your and your practice details to MEDI.ONLINE@humanservices.gov.au
- calling the Medical Certificate Ordering Service on the toll-free number 1800 801 667.
On 1 July 2019, we’ll release a new online Rheumatoid Arthritis form as a feature in HPOS.
Make sure you’re using our current order forms for PBS and RPBS prescription stationery.
