Make sure you include your bank and employer account details

You need to give us your bank and employer details when you apply for a Medicare provider number.

If you’re applying for a Medicare provider number and have indicated that you’ll be claiming a Medicare benefit, you need to give us:

  • your current bank account details
  • details about your employer or contracting organisation

The quickest way to apply for a subsequent Medicare provider number is online through Health Professional Online Services (HPOS). 

We’ve updated and simplified our application forms. You can now use the same form to register for an initial or subsequent provider location number. 

Important: From 9 September 2018, we’ll return applications that don’t include bank and employer details.
