Information for eligible prescribers on how to order prescription forms. These forms are free for eligible prescribers.

About

Eligibility

To be eligible for Pharmaceutical Benefits Scheme (PBS) or Repatriation Pharmaceutical Benefits Scheme (RPBS) prescription forms, you must:

  • have a PBS prescriber number, and
  • have a provider location as registered with us

Ordering PBS and RPBS forms

We only accept typed prescription order forms. Handwritten, incomplete or poor quality forms will be returned to the prescriber.

To make sure you complete the order form correctly:

If you’re ordering personalised stationery, your details are scanned and printed directly from the information provided on your order form.
We’ll contact you if there are issues.

PBS stationery orders can be sent to the prescriber's practice location or a PO Box registered with Medicare. Contact us for more information on how to register or change your preferred mailing address.

Ordering by email

Email completed PBS stationery order forms to PBS.Stationery.orders@humanservices.gov.au where they will be handled and stored securely for processing. Remember to include your prescriber number in the email subject line.

All stationery orders are sent securely to the departments contracted printer for printing and dispatching.

Delivery advice and timeframes

Include your email address on your order form so you can be notified when your order has been dispatched and to get a tracking reference number.

Personalised stationery is dispatched within 28 days of the order being received.

Non-personalised stationery is dispatched within 2 days of the order being received.

Australia Post is the authorised distributor for PBS stationery. See Australia Post for delivery timeframes.

Completing PBS and RPBS prescription order forms

Advice on how to complete PBS and RPBS prescription order forms.

We only accept typed prescription order forms. Handwritten, incomplete or poor quality forms will be returned to you.

Prescriber full name - type your name the way you want it to appear on the prescription form, for example Dr John Smith or Prof Jane Jones.

Prescriber qualifications - these will be printed exactly as you type them, including capitals, spaces and commas.

Prescriber current practice name and address - enter the name of your practice, for example The Health Clinic and the practice address details. This can't be a PO Box.

Prescriber number - add your prescriber number here.

Phone number - enter a mobile number or landline number, including the prefix for the landline phone number. Numbers must be formatted as:

  • landline (02) 9999 9999
  • mobile 9999 999 999

Quantity of books required - select only 1 of the boxes to indicate the quantity required.

Provider number - enter your provider number for the relevant practice location.

Delivery address - enter the name of the prescriber and the registered practice address or PO Box. In the delivery address field, include as many details as possible including level number, suite number, department or shopping centre name. The delivery address must be written in full and be recorded with us. We can’t accept 'As above' in the delivery address field.

  • pharmacist
  • medical practitioner, or

Approved pharmacists Applying for Approval to Supply Pharmaceutical Benefits at a Particular Premises form (PB009) Use this form if you are applying for approval to supply pharmaceutical benefits at a particular premises under section 90 of the National Health Act 1953 in any of the following circumstances:
  • expanding or contracting approved pharmacy premises
  • relocating an approved pharmacy
  • relocating an approved pharmacy (with a change of ownership), or
  • establishing a new pharmacy
Approved pharmacists Authority for Authorised Pharmacist(s) to sign claim forms on behalf of Approved Pharmacist(s) form (PB135) Use this form if you are an approved pharmacist(s) and business owner(s) of a pharmacy approved under section 90 of the National Health Act 1953.
Use this form to:
  • authorise a pharmacist(s) to sign pharmaceutical benefit claim forms and endorse pharmaceutical benefit prescriptions on behalf of the approved pharmacist(s), or
  • request removal of previously authorised pharmacist(s)
Approved pharmacists Authority for Authorised Pharmacist(s) to sign claim forms on behalf of Section 91 Permission Holder form (PB138) As a permission holder under section 91 of the National Health Act 1953, you must complete this form to authorise a pharmacist to sign pharmaceutical benefit claim forms and endorse pharmaceutical benefit prescriptions on your behalf

  • supply pharmaceutical benefits and highly specialised drugs in respect of a private hospital
  • supply pharmaceutical benefits and highly specialised drugs in respect of a public hospital participating in the Pharmaceutical Reforms
  • supply highly specialised drugs in respect of a public hospital not participating in the Pharmaceutical Reforms
  • to claim for the supply of highly specialised drugs in respect of a public hospital where the dispensing of those drugs is undertaken by a third party
Hospital authorities Notification of Bank Account Details for a hospital authority form (PB141) Use this form if you are a hospital authority approved under sections 94 or 100 of the National Health Act 1953 and you wish to:
  • register your bank account details for payments made through Medicare's Online Claiming for Pharmaceutical Benefits Scheme (PBS), or
  • make changes to your existing bank account details
Hospital authorities Authority for Authorised Person(s) to sign claim forms on behalf of a hospital authority form (PB142) Use this form to:
  • authorise a person(s) to sign forms on behalf of a hospital authority seeking approval under section 94 or 100 of the National Health Act 1953 to supply pharmaceutical benefits or highly specialised drugs
  • authorise a pharmacist(s) to sign pharmaceutical benefit claim forms and endorse pharmaceutical benefit prescriptions on behalf of a hospital authority
  • request removal of previously authorised pharmacist(s)

Medicine export declaration form

The Medicine export declaration form (2619) must be included when sending medicines overseas.

PBS patient refund receipt

Authorised pharmacists can issue an official Medicare patient refund receipt Pharmaceutical Benefits Scheme Receipt - S.87A, National Health Act 1953 form (PB011) for PBS items supplied.

Obtaining Medicare numbers

To obtain Medicare or Department of Veterans' Affairs card numbers for use in PBS or RPBS claims, call us.

You will need the patient's consent.

Medicare can release a maximum of 5 numbers and expiry dates per call. For more than 5 numbers, Medicare has established a fax-back service to pharmacists. Use the Medicare number release form to get the patient's Medicare or Veterans' card numbers and expiry dates.

Use the Seemingly Valid Medicare Card form when:

  • the pharmacist has previously submitted a Pharmaceutical Benefits Scheme (PBS) or Repatriation Pharmaceutical Benefits Scheme (RPBS) claim to Medicare for this patient and Medicare found that the Medicare or Repatriation details were invalid and rejected the prescription
  • the patient has presented another prescription and cannot provide any alternative Medicare or Repatriation card details
  • the pharmacist is satisfied that the original details provided by the patient are correct, and
  • the pharmacist has no reason to believe that the patient identified on the prescription is not entitled to receive pharmaceutical benefits

If the above criteria are satisfied, the pharmacist should use the Seemingly valid Medicare card special number, and either:

  • complete and attach the form to the prescription, and retain in paper or electronic (scanned) form for a period of 2 years from date of supply for post payment audit checks, or
  • photocopy or photograph the Medicare card, use the same special number, attach the photocopy or photograph to the prescription, and retain in paper or electronic (scanned) form for a period of 2 years from date of supply for post payment audit checks

Remote Aboriginal Health Services

Use Aboriginal Health Services (AHS) pharmaceutical supplies request form (PB042) to request AHS pharmaceutical supplies for the treatment of patients of the health service.
