To apply for the PIP your practice must be eligible. A practice owner can apply at any time, online or by form.
Practices can apply to join the PIP at any time either:
- through HPOS or
- by completing the Practice Incentives Program Application form and faxing it to us with the required supporting documentation.
Practices must read the guidelines and meet the eligibility requirements for each of the PIP incentives they wish to apply for.
Incomplete application forms may delay the application process.
Practices must provide supporting documentation when applying for Practice Incentives Program.
Practices must provide a copy of their current accreditation, or registered for accreditation certificate and for each practice branch that provides 3,000 or more services per year.
